Step 4: Donation Form

If you wish to mail a letter, please consider enclosing a “Donation Form." Many find that once a person is asked to make a donation, they’ll keep your letter and donation form handy to remind them to sponsor you, even though they may make their donation online. If you are planning on sending an email, make sure to include the link to the donation form. The donation form can be easily printed out and either mailed in with their donation or given to you in person. This will help maximize the donations by avoiding services charges.
